Modifying a Medical Note Template

 

The Template Modification feature is one of the most powerful aspects of NoteDx. It allows users to pre-structure medical notes by creating templates with various sections, from history of present illness to investigations, and even custom instructions for the AI to follow. This is how you get a fully customized and robust note making assistant AI.

 

How to Modify a Template:

 

1. Access the Template Editor: Go to the “Edit Template” section from the app’s main menu on the lower left.

2. Add Sections: From the left-hand sidebar, choose the elements you want in your note (e.g., Identification, Chief Complaint, Medical History, etc.).

3. General Instructions for the Model: Customize a “General Instruction” section. This is critical because it provides the AI with guidance on how to interpret the inputted data. For example:

​

• Instruction for AI: “At the end of the note, in the Impression section, always write a 3 sentence summary that includes the most relevant aspects of this clinical case.”

• You can also specify how detailed you want the AI’s answers to be, whether to focus on specific guidelines, or to ignore certain data.

Anything is possible, and it's time to experiment:


- Ask the AI to flag values:
- If the heart rate is greater than 100, add ** in front of the value...

4. Custom Sections: You can create custom sections tailored to your specific practice needs by clicking on “Custom Section” and defining its structure.

5. Save and Apply: Once your template is structured to your liking, hit “Save”. This template can be applied to any new patient note, ensuring consistent documentation.

 

💡 Tip: Spend time creating detailed, comprehensive templates that match your workflow. For instance, you might create separate templates for cardiology, general surgery, or primary care, each with unique sections and instructions.

 

Writing Instructions for AI

 

When customizing templates, providing clear and specific instructions to the AI enhances its output. Here’s how to structure AI instructions:

 

1. Use Clear, Direct Language: Be explicit about what you want. For example:

  • “Summarize this patient’s clinical history in 3 sentences.”

  • “Do not use bullet points and write complete sentences. Group the HPI per systems.”

2. Specify Length: If you prefer concise responses, mention it. If you’d like detailed suggestions, let the AI know.

• E.g., “Always include a differential diagnosis list with brief explanations (1-2 sentences each).”
